What I understand is that he's looking at an old picture of the woman whom he is still with, but she has changed so much... maybe because she has aged a lot or because she isn't the same person he knew then, personality wise. So, as he looks at the picture, he sees the resemblance and it brings back many good memories of how things used to be but aren't anymore.
